Office Hours:Monday-Friday 8:30 am-5:00 pm

Position Summary:

As a member of an Integrated Dual-Diagnosis Treatment (IDDT) clinical case management team, the Case Manager plays a critical role in supporting a clients progress as they move through the stages of recovery, and assists them to transition from the behavioral health care system. The Case Manager is responsible for establishing contact with newly referred clients and developing a collaborative relationship that will facilitate their participation in treatment. The Case Manager is responsible for maintaining consistent contact with clients, deepening the treatment relationship, assisting clients to identify life goals and linking them to community resources that can support their wellness and recovery. The Case Manager needs to have the skills, knowledge, and ability to shape an effective working alliance, foster hope, build motivation, assess pressing needs, identify barriers to treatment, and link clients to resources to meet their basic needs. The Case Manager must demonstrate the knowledge and clinical expertise to teach clients the skills that will support recovery and enable them to successfully pursue their goals and to provide clients with regular coaching to apply these skills in their daily lives. Successful candidates will be knowledgeable and experienced in employing intervention strategies using motivational interviewing, harm reduction and abstinence-based models of treatment.

Qualifications:

Required:

  • Must have experience in meeting ONE of the following categories:
    • AA degree and six years of experience in a mental health setting.
    • BAdegree and four years of experience in a mental health setting.
    • A certification in Alcohol and Drug Abuse Studies (CATC, CADC, or CODC)
    • Masters plus 2 years of experience in a mental health setting.
    • Must have experience working with adults with the serious mental health symptoms and co-occurring disorders.
  • Must have a vehicle to use for work duties (e.g., be comfortable driving to and from client residences), a valid California Motor Vehicle Operators License, and automobile insurance.
  • Must demonstrate an ability to work both independently and collaboratively as a member of a treatment team.
  • Must be able to maintain a flexible work schedule in accordance with client and program needs.
  • Must have strong organizational skills, as well as, the ability to effectively prioritize competing demands and to meet deadlines.
  • Must have strong interpersonal skills, and the ability to effectively manage clients needs including risk assessment and de-escalation.
  • Experience with Medi-Cal documentation standards strongly preferred.
  • Must be computer competent, including experience with Electronic Health Record (EHR) systems.

Essential Functions/Skills:

  • As a member of the clinical case management team, works collaboratively on a multi-disciplinary team to coordinate treatment and supportive services for up to 30 adults with co-existing psychiatric and substance use conditions in all stages of treatment and recovery.
  • Provide intensive outpatient case management, rehabilitative therapy and support to adults with mental health and/or substance use symptoms.
  • Help organize and lead social and recreational activities to help clients structure their time, increase social experience, and provide opportunities to practice social skills.
  • Must complete individual therapy, biopsychosocial assessments including ANSAs and sound clinical documentation that supports client treatment plans.
  • Actively participate as a team member, and present updates at each weekly team
  • Develop a keen understanding of Welligent and billing codes in order to document and bill MediCal appropriately and effectively assist in training new staff and
  • Assist staff with completing standard documentation requirements as needed and
  • Oversee therapeutic milieu including greeting members, ensuring they sign in/out, engages with members, coordinates daily room use schedule, and maintaining a safe and welcoming
  • Participates in clinical supervision with the agency QI Clinical Supervisor in order to ensure appropriate treatment and sign off on assessments as well as gain hours towards licensure if
  • Assisting at-risk Medi-Cal recipients or potential eligibles to understand the need for MH
  • Actively encouraging reluctant and difficult Medi-Cal recipients/potential eligibles to accept needed MH and healthcare services.
  • Operate a motor vehicle to include safe transport of clients to various appointments across Alameda
  • Non-SPMP gathering information about an individuals health and MH
  • Document client progress to maintain a permanent record of client activity according to established methods and procedures, meeting both Bonita House Inc and County documentation
  • Help clients (and family members) identify, understand, and combat stigma and discrimination associated with mental illness and develop strategies to reduce self-stigma.
  • Collaborate with the team to ensure the protection of clients rights in order to help clients to improve their knowledge of client rights and grievance or complaint procedures; support clients with filing, mediating, and resolving
  • Provide ongoing assessment, problem-solving, side-by-side services, skill teaching, support (prompts, assignments, encouragement), and environmental adaptations to assist clients with challenges in the areas of employment, housing, and activities of daily living.
  • Provides culturally responsive, psycho-social rehabilitation case management services to meet clients comprehensive needs, including linking clients to housing, employment, medical, dental, psychological, parenting, legal, transportation, and money management resources.
  • In collaboration with the client, formulates client-centered, recovery-oriented, integrated treatment plan: assists client in identifying and articulating personal goals; identifies behavioral objectives and mental health symptoms that may be barriers to those goals; determines treatment strategies and interventions to achieve desired goals; establishes measurable outcomes to determine the effectiveness of treatment.
  • Conducts outreach to engage the client in services and treatment, and when appropriate, family members, to support the client.
  • Provides psycho-education to clients and when appropriate, family members, related to health, well-being, and lifestyle, including symptom management, medication, diet, sleep hygiene, trauma-informed care, and relapse prevention; assists clients in the creation of personal WRAP plans, and/or Advance Directive.
  • Provides ongoing review of a clients mental status, response to treatment, and progress in recovery. Initiates modifications to the treatment plan to ensure appropriate interventions are provided in response to changes in the clients mental status, behavior or stage of recovery.
  • Assists clients in developing an individualized transition plan to facilitate exit from behavioral health system or transition to lower level of mental health services.
  • Performs risk assessments, consulting with supervisor and team clinicians as needed, and develops crisis management plans targeting problematic behaviors and delivers effective treatment interventions to reduce the risk of harm to self and/or others.
  • Administrative documentation responsibilities include writing progress notes and individualized service plans and assessments. It is a requirement of the position that consumer records be maintained in a timely fashion according to regulatory standards.
  • Responsible for producing the required number of Medi-Cal billable units per month.
  • Liaisons and consults with community-based organizations to ensure coordination of care.
  • Participates in staff meetings.
  • All other duties as assigned by supervisor.

Physical Activity:

Manual Dexterity & Communication:

Requires the ability to operate a computer, keyboard, and other office equipment; to talk and hear in person and by telephone; and to read printed and digital materials.

Work Environment:

Work is typically performed in an office or community-based setting with standard lighting, temperature, and noise levels. Some positions may require travel between locations as part of regular duties.

Travel required:

This position requires regular travel to multiple agency and partner locations within the Bay Area. Employees must have reliable transportation and the ability to reach designated sites within reasonable travel times.
