We are seeking a dedicated professional to provide high-quality nursing services for high-acuity patients in a supportive ambulatory care setting. At Socorro General Hospital , you will impact the local community while enjoying a weekday schedule and a collaborative environment. Responsible for walk in and telephone triage. Coordinates patients care through collaboration with the patient/family and healthcare team. Provides patient care by assessing, planning, implementing, evaluating and documenting individual patient needs, actions taken and patient responses. Responsible for case management based on PCP panels, in collaboration with network providers to include nurse visits, telephone management, etc. Delegates or assigns aspects of patient care to healthcare team members commensurate with their validated competency to perform the task. Documentation within scope of practice. Performs other functions as required. Qualifications: State of New Mexico or Compact State Nursing License Required Registered Nurse (RN) BLS certification required at time of hire Minimum ADN preparation with expectation of BSN completion within 5 years of hire We're all about well-being, starting with yours.
