Salary Range $21.98 - $24.78 Hourly

Description

Essential Functions:

• Works directly with residential or day habilitation staff under supervision of Behavior Intervention Specialist, Manager, and Coordinator.
• Works with team to look at each person holistically, identifying strengths, preferences, and needs.
• Identifies behavioral, psychiatric, social, vocational, health, nutritional, and mobility needs of each person and brings these to the team's attention.
• Assists in conducting assessments and observations.
• Develops goals/outcomes identified in Comprehensive Functional Assessment or Life Plan, ensuring accurate implementation and progress reports.
• Designs effective data collection systems to monitor and measure progress.
• Ensures all programming is implemented consistently and in accordance with agency policy.
• Monitors completion of habilitation related paperwork and shares reports with appropriate service providers.
• Ensures interventions receive proper approval prior to implementation.
• Helps develop group curricula and ensures implementation of individual and group programming.
• Assists with staff training to ensure proper and consistent implementation of clinical interventions.
• Facilitates case conferences and functions as a member of the interdisciplinary team.
• Participates in weekly interdisciplinary team meetings.
• Functions as a resource to other programs and serves on various committees.
• Supports team when working with families and members of the community.
• Provides clinical information to staff and maintains confidentiality.
• Initiates appropriate referrals for necessary consultations and evaluations.
• Ensures individuals receive appropriate mental and physical health care.
• Facilitates coordination of individual treatment between residence and day hab.
• Maintains habilitation records in accordance with OPWDD and BIRCH policy.
• Brings programmatic and individual clinical issues to the attention of the interdisciplinary team.
• Meets regularly with supervisors to discuss clinical and programmatic issues.
• Functions in a direct service professional capacity.
• Works under the direction of the Residential Manager, Operations Coordinator, Behavior Intervention Specialist, and Registered Nurse.

Qualifications

Education: Bachelor's Degree in Psychology, Social Work, Education, or related Human Services field.

Skills and/or Experience Required:

• Minimum of 3 years' experience working in OPWDD related field, preferably in a residential, health, or treatment related setting.
• Demonstrated competency in written, verbal, and computational skills to document records in accordance with program standards.
• Ability to work a flexible schedule if needed to accommodate working with individuals in the residence and attending team meetings.
