This position supervises clinic staff, coordinates daily patient flow, maintains clinical readiness, supports staff competency, and provides direct patient care within the LVN scope of practice. Reporting to the Director of Urgent Care, the LVN Supervisor works closely with the physician site leader/medical director and on-duty providers to promote consistent protocols, regulatory compliance, patient safety, and high-quality care. The position supports, but does not replace, the provider's responsibility for medical decision-making and clinical oversight. ESSENTIAL DUTIES Direct Patient Care Within LVN Scope
- Perform basic assessment and data collection, including chief complaint, focused history, allergies, medication reconciliation, vital signs, pain screening, and observation of normal versus abnormal findings.
- Support protocol-driven intake and prioritization by recognizing red flags or changes in condition, notifying the on-duty provider, and activating emergency or transfer procedures when indicated.
- Administer medications, injections, immunizations, and skin tests; withdraw blood; and provide IV therapy only when permitted by California law, provider direction, certification, training, competency, and organizational procedures.
- Perform or assist with specimen collection, point-of-care testing, electrocardiograms, wound care, dressings, procedure setup, and other authorized nursing services.
- Assist providers during examinations, minor procedures, stabilization, emergency response, and transfers to a higher level of care.
- Provide patient education and reinforce medication instructions, discharge guidance, follow-up needs, and return or emergency precautions.
- Document care, observations, interventions, education, escalation, and patient response accurately and promptly in the electronic health record.
- Maintain competency in BLS, AED use, emergency equipment, and clinic emergency protocols.
- Ensure completion of assigned opening and closing clinical procedures, including room readiness, medication and temperature logs, emergency equipment checks, laboratory readiness, supply security, and appropriate end-of-day handoff.
- Coordinate safe and efficient patient flow from intake and rooming through testing, procedures, discharge, transfer, or escalation to a higher level of care.
- Monitor wait times, room turnover, incomplete orders, pending specimens, and discharge readiness; escalate barriers affecting patient care or safety.
- Conduct routine readiness checks for exam rooms, medications, supplies, laboratory areas, emergency equipment, infection-prevention controls, and required logs.
- Ensure time-sensitive results, orders, and clinical handoffs are completed or routed to the appropriate provider before the end of the shift.
- Serve as the on-site clinical escalation point for workflow concerns and promptly route medical questions or decisions to the appropriate provider.
- Support urgent care accreditation activities, including readiness assessments, evidence collection, staff preparation, mock surveys, and corrective-action follow-up.
- Monitor adherence to UMMA requirements related to patient safety, HIPAA, OSHA, infection prevention, medication management, laboratory operations, and urgent care accreditation.
- Complete or oversee routine checks of medication and vaccine storage, temperatures, emergency equipment, crash cart/AED readiness, supply expiration, point-of-care testing, specimen handling, and infection-prevention practices.
- Ensure occupational exposures, sharps injuries, and other clinical exposure events are immediately addressed, reported, and escalated in accordance with organizational exposure-control procedures.
- Report and assist with follow-up of patient-safety events, medication or testing errors, exposure incidents, transfers, and other clinical concerns.
- Ensure clinical equipment is clean, functional, maintained, and used appropriately; remove malfunctioning equipment from service and coordinate repair.
- Monitor clinical supplies and inventory and coordinate replenishment with the appropriate operational lead.
- Maintain regular communication with the Director of Urgent Care and physician site leader/medical director regarding clinical risks, incidents, patient-safety concerns, training needs, and unresolved barriers.
- Collaborate with Quality, Compliance, Pharmacy, Laboratory, Radiology, Infection Prevention, Facilities, Information Technology, and other operational partners.
- Support coordinated handoffs to EMS, emergency departments, hospitals, primary care, specialists, and internal care teams as ordered and within scope.
- Provide day-to-day supervision, assign work, coordinate staffing and break coverage, and adjust clinic staff assignments based on patient volume and operational needs.
- Lead clinical huddles and staff meetings to communicate staffing needs, patient volume, safety concerns, clinical priorities, approved updates, and equipment or supply issues.
- Set performance expectations, monitor staff performance, provide coaching and recognition, document feedback, and follow through on improvement plans.
- Participate in recruiting, interviewing, onboarding, probationary reviews, evaluations, and corrective action in partnership with the Director of Urgent Care and Human Resources.
- Provide or coordinate clinical orientation, training, retraining, and competency validation for workflows, equipment, laboratory procedures, medication safety, infection prevention, documentation, emergency response, and patient transfers.
- Maintain required staff training, competency, licensure, certification, fit-testing, exposure-control, and emergency-drill records and address gaps before duties are performed.
- Educate clinic staff on approved clinical protocols and monitor consistent implementation across shifts and rotating providers.
- Collect clinical data and recognize and escalate abnormal findings, but do not independently diagnose, prescribe, establish medical treatment plans, or determine final patient disposition.
- Stop and escalate any task when scope, supervision, authorization, competency, staffing, equipment, or patient safety is uncertain.
- Working knowledge of medical terminology
- Current, unrestricted California LVN license in good standing.
- At least three years of recent experience in urgent care, emergency services, ambulatory care, family medicine, or another fast-paced outpatient setting.
- Current American Heart Association Basic Life Support certification.
- California IV Therapy and Blood Withdrawal certification, or ability to obtain within 90 days of hire before performing those functions.
- Demonstrated competency in medication administration, injections, immunizations, specimen handling, point-of-care testing, infection prevention, emergency response, and electronic health record documentation.
- Knowledge of LVN scope of practice, clinical documentation, patient safety, OSHA/HIPAA requirements, and ambulatory quality standards.
- Basic knowledge of Clinic policies and procedures
- Strong clerical/administrative skills including use of personal computer, especially medical word processing and spreadsheets
- Able to handle multiple-phone line
- Ability to maintain absolute discretion over highly sensitive patient information.
- Very effective people skills including oral communication skills and customer service skills
- Ability to work effectively with people of diverse cultures, ages, and economic backgrounds
- Detail and task-oriented
- Must work well with constant interruptions
- Follow set routines and be alert to variations and make decisions accordingly
- Must maintain organized and accurate records
