Requirements

• Required certification: Pharmacist
• 1+ years of experience
• Key Requirements Summary: 1. Experience: 1 year required; 2+ years preferred 2. Cases: Inpatient/Acute care experience required 3. Certifications: PharmD, RPh, WI pharmacy license, and BLS required; ACLS highly preferred; will accept Act 10 license 4. License: WI pharmacy license required; traveler needs to apply for both temp license and permanent licensure 5. Education: PharmD required 6. EMR: Epic experience required 7. Shifts/Call: 11:00 AM to 11:00 PM hours, 8-hour shifts, every 3rd weekend rotation 8. RTO: Two pre-approved time off periods allowed 9. Returning Staff: Cannot work past 1 year as traveler; 6 month break required to return as traveler 10. Travel/Local: 70-mile radius; locals not accepted 11. Background: Not specified 12. Modules: Non-billable, completed pre-start; RN modules 5-6 hours, Allied modules approximately 1 hour; recorded on transcripts 13. Travel Experience: Required; first time travelers accepted 14. Scrubs: Provided by facility 15. Flu Vaccine Requirements: Unknown 16. Special Equipment: Skills include pharmacokinetic dosing, renal dosing, therapeutic med monitoring, drug shortage management Critical: OSHA Respirator Clearance required ($22 cost). Must shave for fit testing - no exceptions for medical/religious exemptions. CAPRs and PAPRs not provided and personal equipment not allowed.
