Social Work Case Manager

Department: Case Management
Work Hours: Full time 40 hours per week with weekend and holiday rotation and on-call rotation per department needs. Occasional travel between hospital campuses per departmental needs. This position is chiefly responsible for complex transition of care post hospitalization to include psychosocial assessment, consultation, education, community linkage, crisis intervention, and support services for referred patients and families. The position supports the provision of health care services and appropriate utilization of resources across the continuum of care. Requirements:
Bachelor's degree in social work or a bachelor's degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, or psychology.
One year supervised social work experience in a health care setting working directly with individuals.
Basic computer skills.
Successful completion of Acts 73 and/or 169 clearances within 90 days of commencing employment, if applicable. Preferred:
Masters Degree in Social Work; License in Social Work Equal Opportunity Employer
