Minimum Requirements

Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program recognized by the APTA. Active Arizona Physical Therapist license required to start the assignment. BLS certification from the American Heart Association or American Red Cross may be required. Experience in outpatient orthopedic settings preferred, including evaluation, plan-of-care development, and progress toward discharge goals. Proficiency with electronic medical record documentation including goal writing, letters of medical necessity, and charge entry preferred. Ability to supervise and delegate therapy-specific tasks and to participate in student clinical education as needed preferred. Adherence to state practice act, professional codes of conduct, and completion of required continuing education.

Assignment Details

Facility Type: Outpatient clinic. Shift Pattern: 5x8 schedule. Discipline: Physical Therapist, primary orthopedics focus. Duration: 13 week assignment. Responsibilities include comprehensive evaluations, individualized treatment planning, documentation, patient and family education, and promoting patient and employee safety.
