Physical Therapist

Schedule: Days: 8am-5:00pm (Schedule can be flexible based on patient's availability.)

Assignment Details:

• Guaranteed Hours: 40
• Contract Weeks: 13

Requirements:

• One year of Home Health experience

Roles & Responsibilities:

• Evaluate and treat patients following medically prescribed physical therapy programs to restore function and minimize disability.
• Document observations, clinical findings, skilled interventions, goals, and discharge plans.
• Supervise Physical Therapy Assistants as needed.
• Conduct initial admission and evaluation visits.
• Notify assigned Clinical Team Manager of OASIS scores within established timelines.
• Perform other duties as assigned, consistent with skills and training in alignment with the organization's goals and mission.
