Compassionate Hospice RN Case Manager

Gentiva’s Hospice Plus division is seeking a compassionate Hospice RN / RN Case Manager to lead in-home care for patients with life-limiting illness. You will assess patients, administer skilled nursing care, and partner with an interdisciplinary team to craft and update personalized plans of care. You will educate families, provide emotional and bereavement support, and ensure accurate documentation while maintaining safety and regulatory standards. #J-18808-Ljbffr
