Licensed Practical Nurse (LPN) - Assisted Living Facility

Location: Staten Island, NY
Compensation: $40/hr (plus Sign-On Bonus)
Schedule: Monday-Friday, 11:30 AM-8:00 PM

We are seeking an experienced Licensed Practical Nurse (LPN) to join our client, a leader in home health care, in one of their Assisted Living Facilities in Staten Island, NY. In this role, the LPN will perform nursing tasks and responsibilities under the direction of a Registered Nurse (RN), providing compassionate, high-quality care to residents. CHHA experience is preferred.

Responsibilities

• Provide nursing care to residents in the assisted living facility under the supervision of the Registered Nurse
• Carry out assigned clinical tasks and responsibilities in accordance with facility standards
• Monitor residents' conditions and report changes appropriately
• Support residents' long-term care needs and overall quality of life
• Maintain accurate documentation and communication with the care team
• Work independently while ensuring high standards of patient care

Qualifications

• Active New York State LPN license required
• CHHA experience preferred
• Strong ability to work independently with minimal supervision
• Excellent clinical skills
• Superior communication skills
• Basic knowledge of office applications and equipment, including Outlook, Word, printing, and scanning applications
