Job Details Salary: $38,395.00 annually Location: Wayne County, PA (323 Tenth Street, Honesdale, PA 18431) Job Type: Civil Service Permanent Full-Time Union: Non Union Position Overview Located in Honesdale, PA, Wayne County Area Agency on Aging is seeking a compassionate, organized individual to help older adults access services and supports that allow them to remain active, safe, and independent in the community of their choice. Responsibilities Intake Support: Gather information from consumers and provide referrals and explanations of available services Service Coordination: Arrange and monitor services for consumers with simple to complex needs Assessment Work: Conduct initial and annual assessments in homes, facilities, hospitals, or by phone Record Management: Document case activity, complete reports, and maintain accurate files Community Collaboration: Work with families, providers, and community agencies to support consumer needs Travel Requirements: Travel within the county to meet consumers in varied locations and conditions Work Schedule and Additional Information Full-time employment Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 60‑minute lunch On‑call hours when needed Overnight travel or out‑of‑county travel may be required Telework: You will not have the option to telework in this position Salary: In some cases, the starting salary may be non‑negotiable Qualifications Six months as an Aging Care Manager 1 or a County Caseworker 1; or Successful completion of the County Social Casework Intern program; or Six months of professional human services or nursing experience and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training Other Requirements You must meet the PA residency requirement You must be able to perform essential job functions Legal Requirements You must pass a background investigation Conditional offer of employment will require a drug screening This position falls under the provisions of the Child Protective Services Law, which requires submission and approval of satisfactory criminal history reports including PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance EEO Statement The Commonwealth is an equal employment opportunity employer and is committed to a diverse workforce.
