Employment Type: Per Diem / Contract
Licenses/Certifications: State medical license; Board-certified or Board-eligible in Hospice and Palliative Medicine preferred
Experience: Prior experience in hospice/palliative care preferred

What You'll Do
- Provide specialized hospice and palliative care to patients in accordance with facility policies
- Collaborate with the care team to develop and implement patient-centered care plans
- Conduct patient assessments and manage symptoms effectively
- Precept with residents and assist with physician responsibilities as needed
- Support patients and families through complex medical and emotional challenges
- Maintain accurate documentation of patient care and treatment plans
- Participate in team meetings to coordinate continued patient care
What You Need
Required:- Active Virginia medical license
- State certification or eligibility in Hospice and Palliative Medicine
- Strong communication and teamwork skills
- Previous experience in hospice or palliative care
- Experience precepting or teaching residents
- Knowledge of end-of-life care protocols
