Housing/Case Manager

Under the supervision of the Clinical Supervisor, the Housing/Case Manager will provide case management services to a caseload and provide housing assistance within the facility.

Key essential functions include:

• Case Management:
• Provide case management services to a caseload of up to 15
• Manage and develop systems in the facility
• Complete all paperwork for charting as required by funding source
• Conduct groups periodically or as directed by management

• Housing Services:
• Provide Housing Assistance to clients
• Complete Housing Assessments and Referrals
• Complete HRA 2010-E's (CAPS)

Additional functions may include:

• Attend management trainings, team meetings, and staff meetings
• Provide assistance within the facility where needed
• Performs other duties as required by the Clinical Supervisor

Job requirements include:

• Educational/Training Required:
• Either CASAC or Bachelor degree preferred
• GED/High School Diploma or Associates Degree required
• CASAC-T required

• Experience Required/Language Preference:
• Minimum 2 years' experience with population in a residential setting or related field

• Computer Skills Required:
• Proficient in Microsoft Office and related programs
