This role is crucial in assessing patient needs, coordinating care, and ensuring effective communication among all parties involved in patient care.As the HHA Coordinator, you will serve as a specialist in coordinating Home Health Aide (HHA) services between Case Managers, HHAs, and Clients. This role includes overseeing scheduling logistics, compliance documentation, onboarding support, and care implementation processes.
