What You’ll Do Travel to patient homes, assisted living, and care facilities to complete hospice admissions. Perform clinical eligibility assessments and initiate services. Lead goals-of-care conversations with patients and families. Provide clear education on hospice philosophy, services, and expectations. Coordinate with intake, physicians, and the interdisciplinary team. Ensure a smooth, confident transition into hospice care. Maintain accurate and timely documentation. What You Bring Experience in hospice, oncology, palliative care, or high-acuity hospital settings. Strong ability to communicate clearly and build trust quickly. Confident clinical judgment and decision-making skills. Comfort working independently in a field-based role. Reliable transportation and willingness to travel.
