We seek a Part-time License Practical Nurse at our CTH Facility in Hampton!

Responsibilities & Duties for License Practical Nurse:

Implement and maintain Individual Service Plans for consumers.
Conduct ongoing physical and mental health assessments.
Maintain accurate consumer records and monitor safety.
Supervise and oversee individual medical treatment programs.
Administer and deliver prescribed medications as needed.
Educate consumers on medication compliance and potential side effects.
Assess vital signs height weight and monitor medication compliance.
Complete prior authorizations and assist with psychiatric medication needs.
Ensure compliance with agency policies and relevant local state and federal regulations.

Qualifications and Skills for License Practical Nurse:

Graduation from an accredited License Practical Nurse Program.
Current unrestricted LPN license in Virginia or from a Nurse Licensure Compact State.
Minimum of 1 year of LPN experience in a mental health program and/or residential setting.
Minimum of 1 year of LPN experience working with individuals with intellectual disabilities.
Valid drivers license with an acceptable DMV record.
Proficiency in Microsoft Office.
Preferred: Experience with electronic medical records and basic knowledge of Medicaid/Medicare regulations.
