Our organization is at the forefront of integrating orthopedic rehabilitation into homecare, allowing us to meet patients where they are and provide tailored support for their recovery journey.Work EnvironmentThis position is entirely onsite, requiring the Physical Therapist to travel within assigned areas in Santa Rosa, California, to visit patients in their homes. You will operate within a dynamic, fast-paced environment that emphasizes teamwork and adaptability, making each day both rewarding and diverse. You will collaborate closely with other health care professionals in the practice to ensure a comprehensive approach to patient care.Key ResponsibilitiesDeliver evidence-based orthopedic therapy directly in patients' homes, focusing on rehabilitation after injury, surgery, or for chronic musculoskeletal conditions.Collaborate with in-house medical doctors, occupational therapists, and chiropractor colleagues in developing integrated care plans tailored to each individual's needs.Conduct thorough assessments to evaluate patients' safety, mobility, and functionality within their home environment, and adjust treatment plans as necessary.Educate patients and their families about therapeutic exercises, proper body mechanics, and adaptive strategies to encourage independent recovery.Utilize secure and user-friendly electronic medical record (EMR) tools to document each therapy session and track patient progress.Engage in interdisciplinary case reviews and participate in continuous professional development through ongoing clinical education opportunities.Patient Population / Client FocusThe role will primarily involve working with orthopedic patients recovering from various conditions, including post-operative rehabilitation, traumatic injuries, and chronic pain syndromes.
