Hospice Nurse Practitioner for Pediatric Patients – New York, NY, US Job Description We are looking to hire talented Pediatric Nurse Practitioners for rewarding roles in pediatric hospice care. Our hospice and palliative care programs offer a broad range of services in the community or facility‑based settings to help patients and their families ease the burden of managing a life‑limiting illness or end‑of‑life care. The Pediatric NP Provider in Hospice/Palliative Care has two primary responsibilities: Provide NP‑appropriate services to patients enrolled in hospice and assist in staffing pediatric palliative care consultation services that function as a program of Hospice and Palliative Care. Work in community, long‑term care environment, or hospital settings. Apply specialist skills in symptom management, communication including goal‑setting discussions, and hospice eligibility review; work independently or as part of a treatment team. Provide in‑person or telehealth visits, as appropriate, abiding by best practices for history‑taking, examination, formulation, care planning, treatment recommendation, communication, and documentation. Coordinate with the team physician and manager; conduct face‑to‑face evaluations for hospice eligibility and assess/manage hospice patients requiring urgent medical care. Contribute to training of the hospice team, providing support for the interdisciplinary team as needed. Support building a teaching curriculum on pediatric aspects of hospice care for hospice workers engaged in pediatric care or when young adolescents are integrated into adult hospice teams. Use an evidence‑based approach to management and to the use of materials and policies/procedures while providing both direct care and support to other clinicians. Participate in quality‑improvement projects as appropriate. Qualifications At least 1 year of Pediatric Nurse Practitioner experience as a provider. Active New York State Provider license in good standing. Experience in oncology, hospice, or palliative care is highly desired.
