Position: Ambulatory Registered Nurse (RN) Case Manager

Location: Juneau, AK

Schedule: Monday - Friday, 8:00 AM - 5:00 PM

Job Description

The Ambulatory Registered Nurse (RN) Case Manager provides comprehensive, patient-centered nursing care in an outpatient clinic setting. This role supports individuals and families across the continuum of care by utilizing nursing processes to assess, plan, implement, and evaluate care. The RN will administer medications and treatments, perform clinical procedures, and provide patient education and care coordination in collaboration with providers and interdisciplinary teams.

Key Responsibilities

• Perform comprehensive patient assessments and develop individualized plans of care.
• Implement and evaluate nursing interventions and patient responses.
• Administer medications and treatments safely and accurately per provider orders and policies.
• Recognize changes in patient condition and initiate appropriate interventions.
• Provide patient and family education related to care, medications, and discharge needs.
• Deliver culturally responsive, patient-centered care.
• Collaborate with providers and interdisciplinary team members to coordinate care.
• Facilitate referrals, follow-up care, and continuity of services.
• Document assessments, interventions, and outcomes accurately in the electronic health record.
• Advocate for patient care and assist patients in navigating the healthcare system.
• Participate in quality and performance improvement activities.
• Report adverse events, near misses, and unsafe conditions per policy.
• Float to other departments or assignments as needed.

Required Qualifications

• Current full and unrestricted Registered Nursing license in Alaska or other U.S. state.
• Graduate of an accredited school of nursing; Bachelor's degree preferred.
• Basic Life Support (BLS) certification required.
• Case Management certification preferred.
• Case management experience in a clinic or hospital setting required within the last 2 years.

Professional Expectations

The Registered Nurse must demonstrate professionalism, accountability, and teamwork. They should provide appropriate guidance and delegation to ancillary staff, and support the orientation, mentoring, and education of staff, students, and peers.

This role will be in a fast-paced ambulatory setting, ensuring adherence to infection control and regulatory standards while promoting quality outcomes and patient safety.
