Soliant is seeking a compassionate and dedicated Social Worker for a flexible, part-time opportunity in the Bridgewater, New Jersey area. This position is ideal for a professional looking to maintain work-life balance while making a meaningful impact through case management services.
Position Details
- Part-time position (approximately 2 days per week)
- Flexible schedule—you choose the days that work best for you
- Bridgewater, NJ area
- Primary focus on case management and client support
Responsibilities
- Provide case management services and ongoing support to assigned clients.
- Conduct assessments and develop individualized service plans.
- Coordinate with families, community agencies, and other professionals to connect clients with appropriate resources.
- Maintain accurate and timely documentation.
- Monitor client progress and advocate for needed services.
- Participate in team meetings and collaborate with interdisciplinary staff as needed.
Qualifications
- Bachelor's or Master's degree in Social Work (BSW/MSW) or a related field.
- New Jersey Social Work license (LSW or LCSW) preferred, if applicable.
- Previous case management experience preferred.
- Excellent communication, organization, and interpersonal skills.
- Ability to work independently and effectively manage a flexible schedule.
