Work Environment\n

This position is entirely remote, allowing for flexibility while delivering essential counseling services. The successful candidate will engage with clients via telehealth platforms to provide accessible support, ensuring that clients receive the care they need from the comfort of their homes. This role will also involve collaboration with a multidisciplinary team to develop and implement effective treatment plans.

Key Responsibilities\n

• Conduct comprehensive assessments to identify the counseling needs of clients, utilizing evidence-based tools and techniques.

• Assess and diagnose clients for substance use issues using the DSM criteria, determining the most appropriate level of care in line with ASAM guidelines.
\n
• Prepare the substance abuse counseling component of individualized client treatment plans, incorporating their unique histories and treatment goals.
\n
• Develop client discharge and continuum of care plans to facilitate transitions to subsequent levels of treatment as needed.
\n
• Maintain accurate and thorough documentation of all counseling sessions, assessments, and follow-up care in clients' clinical records.
\n
• Manage a caseload of clients, ensuring timely updates on case management and treatment plans.
\n
• Coordinate support services for clients and their families, clearly recording services rendered in clinical records.
\n
• Engage with external referral sources including legal, educational, and occupational entities to enhance client support.
\n
• Facilitate interventions for crisis-oriented issues through a collaborative, multi-disciplinary team approach.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker at Great Valley Psychotherapy, you will primarily support clients grappling with addiction, co-occurring disorders, and mental health challenges within a hospice care context. You will provide counseling that aims to enhance emotional well-being, coping strategies, and resilience while navigating the complexities of recovery and healthcare.

• A Master’s degree in Social Work (MSW) and valid LCSW license in New York.

• Demonstrated professionalism, integrity, and strong therapeutic boundaries.

• Proficient in electronic health record (EHR) systems and teletherapy platforms.

Preferred Experience\n

Compensation and Benefits\n

The salary for this position ranges from USD 70,000 to 100,000 per year, commensurate with experience. In addition to a competitive salary, we offer a comprehensive benefits package, including:
