Legacy Healthcare Services is searching for an experienced and motivatedPhysical Therapist (PT) to join our newly created Better at Home team. Were committed to providing comprehensive healthcare services in the comfort of our patients homes to positively impact quality of life by redefining the delivery of join a team that is empowering senior adults to lead a life filled with purpose safety and joy!

Pay Rate:Based on a Per Visit Rate
Coverage Location: Tampa FL

Responsibilities:

Provide physical therapy services to patients according to physicians plan of care and as defined in the state Physical Therapy Practice Act.
Assess and evaluate therapeutic rehabilitative/functional status to develop implement and direct physical therapy programs that focus on rehabilitation.
Evaluate the patients home environment assess patients for muscle strength mobility gait ROM potential for rehab.
Direct patients and families/caregivers in the use and care of therapeutic appliances.
Initiate physical therapy program and instruct other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient.
Identify patient and family/caregiver needs for other home health (PCA) services and refer as necessary.
Prepare and submit progress summaries based on the attainment of goals.
Supervises PTAs according to organization policy and state regulations.

Position requirements:

• Degree in area of discipline
• Current Physical Therapy license in state of practice or ability to obtain prior to beginning employment
• Excellent interpersonal fiscal and clinical skills
• Computer proficiency

Pay Range

USD $40.00 - USD $50.00 /Hr.

Required Experience:

IC
