Delivers patient care by incorporating the tenets of professional practice and through the application of the nursing process based on theory and evidence-based practice. Assesses and evaluates patient care needs while applying critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. Prioritizes all aspects of patient care, including teaching, rounding, coaching, and planning after hospital care, and delegates to others as appropriate. Educates patients and caregivers about their plan of care, health promotion, and disease prevention. Communicates effectively and professionally with patients, families, and all members of the patient care team. Manages assignments within the care delivery model and demonstrates effective allocation of resources. Role models patient-centered care through effective actions such as shift reports, medication reviews, and purposeful rounding. Considers the patient's values, preferences, and cultural diversity in all aspects of care. Establishes and maintains therapeutic relationships with patients and families. Uses evidence-based practices to enhance understanding of patients' perceptions of care. Handles patient concerns by initiating service recovery and escalating as needed. Demonstrates compassion, empathy, and active listening through communication. Conducts purposeful hourly rounding addressing patient care needs. Practices safe patient-centered care and identifies barriers to improvement. Demonstrates clinical skills crucial for quality and safety in the patient population served. Implements process improvement strategies based on best practices. Engages in formal and informal peer review processes. Collaborates with teams to create and implement quality improvement projects and activities. Contributes to compliance measures and implements high reliability principles. Documents nursing practice accurately to support quality initiatives. Facilitates patient throughput effectively. Contributes to a supportive care environment for colleagues, patients, and families. Advances clinical competence in nursing practice. Projects a professional image and communicates with respect and compassion. Incorporates ethical principles into decision-making for patients and families. Encourages lifelong learning and innovation for self and others. Participates in decision-making through committees and unit initiatives. Experience with EPIC is highly preferred.
