Licensed Practical Nurse (LPN) - LPN

CA LVN – Urgent Care

Rotating shifts:

• Mon–Fri: 11:30–22:00
• Sat–Sun: 08:30–19:00
• Weekend requirement: Yes, every other weekend; includes some holidays

Requirements:

• Active CA LVN license
• CA IV certification
• BLS certification (AHA or ARC only)
• 1 year experience in Urgent Care or ER
• 2 years total outpatient experience
• Skills: IV start/maintenance, phlebotomy/blood draws, wound care, vaccine administration, vital signs, oxygen administration, basic EKG/12-lead acquisition, medication administration (IM/SQ), foley catheter insertion (male/female), and telephone triage
• Charting system: EPIC

Compliance / Notes:

• Valid permanent CA LVN and CA IV certification required at submission; temporary licenses not accepted
• Two references from within the past year required at submission (must be supervisor or higher)
• Background: Any license/background issues require a written statement; all issues must be closed
• No scheduling requests accommodated; no RTO within first 2 weeks; max RTO 7 days; pre-approved time off allowed: one
