Position Summary

Unison Health seeks a Substance Abuse Specialist to join our Assertive Community Treatment (ACT) Program, an evidence-based model of delivering comprehensive community-based behavioral health services to adults with serious and persistent mental illnesses who have not benefited from traditional outpatient treatment. The ACT model utilizes a multidisciplinary team of providers who work closely together delivering services designed to meet the needs of the clients served.

Key Responsibilities & Role Highlights

• Provide services to assist in resolving or minimizing the effects of a client's impairment.
• Complete assessments, monitor conditions, and conduct crisis interventions.
• Provide education, link and train clients in community resources.
• Assist in daily living skills and help clients obtain necessary services for meeting basic human needs and entitlements.
• Participate as a member of the ACT treatment team and serve as a consultant to the team in the area of substance abuse.

Education & Experience Requirements

• QMHS 3+ with high school diploma required.
• Bachelor’s degree preferred.
• CDCA (can be obtained within 6 months of hire).
• Experience in integrated dual disorders treatment preferred.
