Job SummaryAs an RN Recertification Nurse, you will be instrumental in coordinating, re-evaluating and confirming the medical necessity of continued home health care services for our patients, and providing patient care and ensuring optimal outcomes.With core skills in managed care, patient assessments and compliance, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.You will submit updated documentation of the patient's condition and ongoing care needs timely and within compliance of all state, federal, and agency guidelines.You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.Responsibilities/Requirements of an RN Case Manager / Clinical Supervisor include:RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.RN to Document/Communicate/Coordinate patient status/change in condition to care team.RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.Willing to travel surrounding counties.Minimum of one (1) years of nursing supervisory experience.
