Job Description

• Conduct thorough evaluations for patients in both inpatient and outpatient settings, crafting personalized treatment plans
• Deliver physical therapy interventions that prioritize mobility, strength, balance, and overall functional capabilities
• Coordinate with nurses, physicians, case managers, and an array of rehabilitation professionals to enhance patient outcomes
• Guide patients and their families through therapeutic goals, discharge plans, and prescribed home exercises

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• Preferred CPR certification
