Description

• Conduct evaluations for patients in both inpatient and outpatient environments, creating tailored treatment plans
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to attain optimal patient outcomes
• Provide education to patients and their families regarding therapeutic goals, discharge planning, and exercise regimens for home
• Meticulously document evaluations, treatments, and patient progress as per regulatory and facility standards
• Engage in interdisciplinary team rounds and care conferences for coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• Certification in CPR preferred
• Specialization certification (e.g., neurology, geriatrics) is a bonus but not mandatory

Skills

• Prior experience is strongly preferred
