What You'll Do Float between home health territories throughout the Twin Cities based on patient needs. Complete skilled nursing visits, OASIS assessments, and individualized care plans. Coordinate care with physicians and interdisciplinary team members. Support local teams during census growth, vacations, and coverage needs. Deliver compassionate, one-on-one care that helps patients remain safely at home. Maintain timely and accurate clinical documentation.

What We're Looking For Active Minnesota Registered Nurse (RN) license. Strong clinical judgment and adaptability. Comfortable working independently in a field-based environment. Home health experience preferred, but not required. Reliable transportation and willingness to travel throughout the metro.
