Social Worker
Job Summary:
We are seeking a compassionate and dedicated Social Worker to join our team at Skilled Nursing Facility in Freehold, NJ. Responsibilities:
Assess patient needs and develop individualized care plans to address social, emotional, and psychological concerns
Collaborate with healthcare teams to identify and address patient and family concerns, providing emotional support and guidance
Document patient progress and maintain accurate records
Develop and facilitate patient and family education programs to promote health and wellness
Participate in interdisciplinary team meetings to share knowledge and best practices
Provide bereavement support and counseling to patients and families
