Primary Care Physician

Role: The Primary Care Physician (PCP) provides direct patient care in outpatient and occasionally in inpatient, nursing, or home settings. Responsibilities include conducting geriatric assessments, physical exams, diagnoses, developing care plans, health education, referrals, documentation, and participating in clinical rounds and conferences. The PCP collaborates with healthcare professionals, ensures compliance with standards, and manages resource utilization and discharge planning. They must demonstrate independence and teamwork, handle complex clinical situations, and advocate for patient needs. Qualifications & Skills: Must hold an MD or DO in internal medicine, family medicine, geriatrics, or related field, with an active medical license. Board certification or eligibility is preferred. Requires DEA license, BLS certification, and ability to interpret diagnostic tests, develop treatment plans, and communicate effectively. Leadership, quality improvement, practice inquiry, and technology literacy competencies are essential. Key Duties Assess and manage acute and non-acute conditions independently. Develop and document comprehensive care plans, order diagnostics, medications, and referrals. Participate in team-based patient care, clinical rounds, and conferences. Ensure adherence to quality standards, regulatory compliance, and documentation accuracy. Collaborate with multidisciplinary teams and advocate for patient education and resource access.
