Speech Language Pathologist (Part time)

Overview This is professional, clinical work involved in the planning and provision of treatment services to patients exhibiting communication disorders. Performance is evaluated by the Rehab Manager and/or designee through direct observation, written reports and consultation. Responsibilities Assess SLP care needs after a referral; perform an accurate history and physical exam. Document assessment data within 24 hours of home visit. Describe assessment and interpret data holistically (psychological, physiological, cognitive and medication regimen). Observe reimbursement eligibility criteria in implementing services; assist in obtaining information to determine alternative payment sources. Communicate with insurance companies or HMOs to determine eligibility for home care. Determine priorities of client care based on client needs and available resources; manage time effectively and prioritize client needs. Develop written plan of care that best meets the client\'s needs holistically; define the coordination of care needed; plan includes specific SLP interventions; make referrals as indicated. Establish measurable goals; plan revised and updated every 60 days or 90 days for hospice, as the client\'s condition changes. Cooperatively assist clients and significant others in developing a realistic plan of care. Develop patient plan of care and supervise client\'s care on an individual basis as applies to Assistants/Aides regarding rehab. Provide treatment interventions safely and competently; deliver care according to the plan and established priorities. Practice in accordance with SHC approved procedures, protocols and standards of care. Plan and complete home visits in a timely manner; notify supervisor when deviations occur outside the norm. Meet time management and productivity standards (visits per week as applicable). Demonstrate competency in knowledge and application of SLP skills; interpret physician orders correctly and carry them out as directed. Document using proper format including assessment, interventions, plan and goals; ensure documentation meets Medicare, Medicaid and other requirements. Provide information to client and significant others regarding diagnosis, interventions and available resources; function as a family advocate while protecting privacy and confidentiality. Investigate and follow up to ensure needs are met; utilize appropriate telephone skills to provide rehab care consultations per SHC guidelines. Complete agency documentation (daily schedules, time sheets, mileage, unusual occurrences, employee injuries, paid time off) per SHC guidelines; complete discharge summaries. Assess, plan, implement and evaluate educational needs; develop individualized teaching plans tailored to client goals; implement and document teaching plans; provide practical teaching materials for home use. Modify plans after evaluation as needed; assist patient and caregivers in obtaining equipment and supplies necessary to provide quality care; encourage compliance with therapeutic rehab program. Assess client for risk behaviors and identify risk factors; develop and implement a risk reduction plan with client involvement; promote preventive care and health promotion during home visits; promote client self-care and independence. Provide a safe environment and adhere to SHC safety standards; demonstrate knowledge of and adherence to policies for fire and disaster plans; comply with mandated in-services; safely operate equipment; maintain CPR certification. Identify potential hazards and participate in resolutions; review policies and procedures as they pertain to individual disciplines. Serve as point person for individual disciplines\' issues in the manager/supervisor\'s absence; assist with staffing, clinical questions, registry staff orientation, etc.; review new employee paperwork during the first month of employment. Participate in Utilization Review committee and complete assignments; assist manager in interviewing process for SLP, OT and PT clinical skills and competency. Qualifications Required – Master\'s Degree in Speech-Language Pathology or equivalent education/experience. CA: Required – must complete the education to attain a Speech Therapy license in the state of California; Master\'s Degree in Speech-Language Pathology, Communication Disorders, or related field. Preferred – 1 year clinical experience as a Speech-Language Pathologist. Preferred – Home Health experience as a Speech-Language Pathologist. Certifications, Licenses, and Registrations CA: Required upon hire – California Speech-Language Pathologist License (Vendor Managed).
