Key Responsibilities

• Conduct thorough evaluations and follow-up appointments for 16-20 cardiology patients daily, ensuring every patient's cardiovascular health is meticulously monitored.
• Administer and interpret stress tests, leading to accurate diagnoses and timely treatment plans.
• Develop and implement chronic disease management strategies tailored to individual patient needs, focusing on improving long-term health outcomes.
• Collaborate with a multidisciplinary team of healthcare providers, including physicians, nurses, and clinical staff, to deliver comprehensive care.
• Document patient information and treatment plans using our electronic health record (EHR) system accurately and efficiently.
• Educate patients on heart health, lifestyle changes, and medication adherence to promote overall wellness.

Patient Population / Client Focus

In this role, you will primarily serve a diverse patient population with varying degrees of cardiovascular issues. Your expertise will enable you to cater to both acute and chronic conditions, making a significant impact on your patients’ quality of life. You will work closely with patients of all ages, providing support and education to empower them to manage their cardiovascular health effectively.

Qualifications

• A Master of Science in Nursing (MSN) with board certification as a Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP).
• An active DEA license is required for prescribing medications.
• A minimum of two years of experience as a Nurse Practitioner, preferably within a cardiology or critical care setting.
• Strong clinical skills, excellent communication abilities, and a passion for patient-centered care.
