Work Environment

This position is based onsite within a home care facility, where the Nurse Practitioner will have the opportunity to provide personalized healthcare services directly in patients’ homes. The work schedule is Monday to Friday from 9 AM to 5 PM, allowing for a balanced work-life experience. Our team is equipped with the latest tools and resources to support effective patient management, making sure each practitioner has what they need to excel.

Key Responsibilities

• Conduct thorough patient evaluations, collecting complete medical histories and performing comprehensive physical exams to establish accurate diagnoses.
• Utilize diagnostic tests to assess and manage a wide range of medical conditions, including acute and chronic illnesses, thereby developing individualized treatment plans centered on evidence-based practices.
• Prescribe necessary medications and therapies, ensuring that patients are informed about their treatment options, potential side effects, and expected outcomes to promote shared decision-making.
• Deliver patient education and counseling tailored to individual health conditions, preventive strategies, and lifestyle changes to enhance health and wellness.
• Collaborate effectively with physicians, specialists, nurses, and allied healthcare professionals to ensure coordinated patient care and respond to healthcare needs promptly.
• Promote preventive care through routine screenings, immunizations, and health education initiatives, emphasizing the importance of preventive measures in reducing the incidence of illness.
• Maintain meticulous Electronic Health Records (EHR) to document patient interactions, adhere to HIPAA regulations, and support the continuity of care.

Patient Population / Client Focus

You will be primarily working with a diverse patient demographic, including individuals across the lifespan who may present with both acute and chronic health issues. This role requires an empathetic approach to patient interactions, ensuring that care is accessible and appropriate for all cultural and age-related needs. Your expertise will enable you to address a variety of conditions, improve patient awareness, and empower individuals in managing their health effectively.

Qualifications

• A Master’s or Doctoral degree in Nursing from an accredited Nurse Practitioner program is required.
• Current and unrestricted Nurse Practitioner license in the state of Maryland is mandatory.
• Certification in a specialty area of Nurse Practitioner practice such as Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (AGNP), or Psychiatric Nurse Practitioner (PMHNP) is highly desirable.

Preferred Experience

• A minimum of 2 years of clinical experience as a Nurse Practitioner is strongly preferred, although new graduates with relevant clinical training and a strong commitment to patient care are encouraged to apply.
• Demonstrated ability to make sound clinical judgments, manage a diverse range of health conditions, and provide holistic patient care.
• Proficiency in using EHR systems (such as Epic or Cerner) and medical office management software is advantageous.

Compensation and Benefits
