Medical Social Worker

Schedule

Day shift, 5x8 hours (08:00 - 16:30).

Job Description

The Medical Social Worker will provide acute medical social work services in collaboration with healthcare team members to support patient care and facilitate discharge planning. Responsibilities include assessing patient needs, coordinating care, and participating in treatment planning.

Responsibilities

• Conduct needs assessments to identify social, emotional, and environmental factors affecting patients.
• Develop comprehensive plans of care in alignment with patient and caregiver needs.
• Engage in collaborative care practices with interdisciplinary teams.
• Facilitate discharge planning and coordination of necessary resources.
• Organize and participate in family meetings and care conferences.
• Provide guidance and information regarding community resources and entitlements.

Required Experience / Certifications / Licensure

• Minimum of 2 years of acute medical social work experience.
• Experience with discharge planning and the dual role of social work.
• Required certifications: MSW or LCSW.
• Current CA state license preferred; pending licenses accepted if active upon start.
