Position Qualifications:

• Education: Bachelor's Degree Required, degree in Public Health and/or Social Work preferred.
• Experience: Previous experience working in patient navigation, case management, or the medical field is preferred. Experience with or strong understanding of the LGBTQ+ community's healthcare needs.
• Skills & Core Competencies: Strong communication skills and people skills to work with patients and our team. Strong problem-solving skills for navigating challenges. Good time management is important to prioritize tasks and meet deadlines. Ability to work independently and as part of a team. A positive, professional attitude that helps build a strong team. Experience with Microsoft Office, especially MS Excel. Spanish proficiency is preferred.

Salary commensurate with experience within posted range: $42K - $60K annually
