What You’ll Do

• Provide hospice and palliative care services to adult patients
• Conduct admissions, eligibility assessments, and recertifications
• Manage symptom control, medication prescriptions, and clinical documentation
• Collaborate with interdisciplinary team members and participate in biweekly IDT meetings
• Support patient comfort at end-of-life, ensuring respectful and compassionate care
• Document care and complete death certificates as needed
• Optional remote work after initial onsite orientation

What You Need

Required:

• WI Medical License
• DEA registration (likely required)
• Board certification in Hospice and Palliative Medicine preferred; internal medicine or hospitalist background considered
• Hospice experience strongly preferred
• Ability to work Monday–Friday, 8:00 AM – 4:30 PM

Preferred:

• Previous experience in hospice/home-based care
• Strong collaboration and communication skills
