RN Care Manager - LTSS & Care Coordination (Travel-Ready)
Molina Healthcare
Molina Healthcare is seeking a Care Manager RN to support care management and care coordination for LTSS. You will collaborate with a multidisciplinary team to ensure high-need members receive quality, cost-effective care across the continuum, including waiver services and home visits as required.
The role requires at least 2 years in healthcare, an active RN license, and experience with waivers, behavioral health, and LTSS.
