**Required Experience and Qualifications:**- PT Experience: Required- Outpatient Experience: Required, primarily outpatient with acute secondary- Permanent Address: Facility will not consider local candidates (50-mile radius restriction)- Work History- Skills Checklist, pertinent to the specialty (Physical Therapy), current to 1 year: Required document- 2 Supervisory Reference Forms completed within the last two years: Required document- Active American Heart Association BLS Certification: Required document- Active Michigan Physical Therapist License as stated by LARA: Required document**Certifications:**- BLS (AHA)**State Licenses:**- Michigan
