divh2Licensed Practical Nurse (LPN) - ElderONE Part Time - Varied/h2pThe Licensed Practical Nurse is responsible for providing direct patient care, usually under the direction of a Registered Nurse, in accordance with established plans./ppThis position has a designated home base facility and may require occasional travel to additional locations. Typically, travel is limited to one to two locations per day, with up to four sites in Rochester, NY and one in Newark, NY. Mileage reimbursement is provided./ppResponsibilities include conducting and documenting a thorough observation of each patients medical status upon admission and throughout the patients course of treatment, assisting in the implementation of the patients treatment plan under the direction of the RN, providing routine nursing services for patients as directed by the RN, complying with the evaluation, treatment, and documentation guidelines of the department, completing required documentation in an accurate and timely manner, providing regular patient status updates to the appropriate personnel, attending and participating in department/facility meetings as required, acting in compliance with hospital, regulatory, and professional standards and policies, maintaining clinical competency by pursuing continuing education opportunities, and performing other duties as assigned./ppRequired licensure/certification skills include a graduate from an accredited School of Practical Nursing, current LPN licensure in the State of New York, and Basic Life Support (BLS) Certification from the American Heart Association (AHA)./ppPreferred qualifications include one year experience and one year of experience working with a frail or elderly population preferred./ppPhysical requirements include medium work, exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; requires frequent walking, standing or squatting./ppPay range is $27.75 - $33.75./ppRochester Regional Health is an Equal Opportunity Employer.
