FamilyCare Health Centers is looking for a Referral Coordinator at our St. Albans Location

Referrals 

Physical Requirements:

The employee is regularly required to talk and hear. The employee is frequently required to stand, walk, sit, stoop, and reach. The employee must frequently lift and/or carry up to 20 pounds and occasionally lift and/or move 40 pounds or more. Specific vision abilities required by this job, include close vision, distance vision, color vision, depth perception, and the ability to adjust focus. The employee could be exposed to bodily fluids, communicable diseases, toxic substances, medicinal preparations, and other conditions common to a clinic environment.

Qualifications: 

High School graduate or equivalent that has experience in computer data entry and medical terminology. Medical assisting certification is preferred.

Duties and Responsibilities:

• Assist providers in offering comprehensive health care services for our patient by coordinating the patient's care with other health care professionals and facilities. 
• Arrange patient care that may include x-ray, imaging, procedures, laboratory tests, specialty and sub-specialty care, counseling, home health or other supportive services, that must be provided at a facility outside of FamilyCare.
• Ensure that all referral orders will be tracked and tied to the corresponding report when it is received to ensure that FamilyCare obtains complete information for the patient record.
• Communicate referral information to patients, specialist, office staff, providers, and payer sources and keeps patients and their health record accurately updated.
