Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: 06/01/2026 Duration: 13 weeks Hours: 30 hours per week Shift: 6 hours Employment Type: Travel Setting: SNF / LTC A great SNF / LTC is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contract assignment. Responsibilities Assessment of referred patients requiring rehabilitation services, including direct patient care, evaluating medical condition, functional capabilities, limitations and potential for rehabilitation. Establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. Set realistic and achievable goals, document and record patient condition, and educate patients and families on appropriate physical therapy methods. Requirements Graduated from an accredited school. Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT. Current CPR certification. Develop a plan of care for each physical therapy patient. Provide skilled physical therapy services/interventions in accordance with physician orders. Assess patient needs, plan for, evaluate and modify care to meet goals of physical therapy interventions. Collaborate with all disciplines to plan and evaluate team goals for each patient.
