Minimum Qualifications:

• Education: Bachelor's degree with progressive experience or a Master's degree in Physical Therapy.
• Licensure: Must be licensed to practice as a Physical Therapist in Illinois.
• Experience: Previous experience in home healthcare or familiarity with conditions of participation is preferred.
• Additional: Must have a current driver's license and reliable transportation to travel to and from patient sites.
