About the Position

Key Responsibilities:
Coordinate discharge planning and patient needs assessments
Manage patient plans of care and DME orders
Ensure compliance with CMS (Centers for Medicare & Medicaid Services) standards
Collaborate with interdisciplinary teams for smooth transitions of care

Requirements

✅ Minimum 2 years of recent RN experience
✅ Acute hospital background
✅ Experience with ICU, MS, PICU, SDU, PCU, IMC, and Observation units
✅ Previous Charge Nurse experience
✅ Strong Case Management & Utilization Review skills
✅ BLS Certification
✅ EPIC EMR proficiency
