Registered Nurse II, Primary Care at Home

ChristianaCare's Primary Care at Home is seeking an experienced Registered Nurse II to deliver high-quality, safe, evidence-based patient care. This role is responsible for achieving optimal patient outcomes through independent practice and collaborative teamwork within the clinical setting. The Registered Nurse II will apply the Nursing Process to assess, plan, implement, and evaluate care, contributing as a trusted and integral member of the healthcare team. Our dedicated team delivers high-quality, patient-centered care directly in the home. Patients are supported by experienced and compassionate professionals, including physicians, nurse practitioners, physician assistants, and nurses, who provide in-home medical visits. Through regularly scheduled appointments, our team builds meaningful relationships with patients, caregivers, and families - helping to enhance comfort, trust, and overall quality of life for homebound individuals. This role is split between triaging for the practice as well as home visits. Primary Care at Home | ChristianaCare Work Schedule Monday through Fridays: 8am to 4:30pm Location North Wilmington in the areas of 19810, 19809, 19703 (Claymont), and 19806 in the city of Wilmington. Duties / Responsibilities Provide age-appropriate, patient-centered care to pediatric through geriatric populations in both virtual and in-person settings. Deliver services such as wellness visits, care management, point-of-care testing, and triaging patients. Collaborate closely with providers to support examinations, procedures, and efficient clinic operations. Serve as a liaison among patients, providers, and staff, ensuring clear communication, coordinating workflow, and responding to urgent situations. Support staff development through training and mentorship. Participate in quality and compliance initiatives and contribute to process improvement efforts. Maintain a strong focus on safety, patient education, and high-quality care delivery. Education Requirements A graduate of a state-approved or National League of Nursing accredited school of nursing. Registered Nurse licensure or eligibility for licensure in the state of Delaware, Maryland, Pennsylvania, or New Jersey depending on location of employment. Two years’ experience as a Registered Nurse; physician office experience is preferred. Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) considered with an agreement to complete a BSN in 3 years. Basic Life Support (BLS) required. Proof of a valid driver’s license and active auto insurance required. Hourly Pay Range $38.22 - $61.16. This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting.
