Job Summary:
The organization is seeking a Home Health Registered Nurse (RN) in Newton, Massachusetts. This role involves assessing patient conditions, developing and implementing care plans, and providing education and support to patients and their families within the scope of community health practices. Candidates are required to bring their license on the first day.

- Required Qualifications: Valid nursing license; at least 1 year of Certified Home Care experience; at least 1 year of Case Management experience; ability to travel to cases as assigned; willing to work on a scheduled basis including weekend rotations once every 6 weeks.

- Float: Float rotation may be required.
- Additional Information: Applicants must have experience within a certified home health agency; hospice, private care, or roles solely as a Case Manager do not meet the qualification requirements. The role requires thorough electronic documentation and interdisciplinary communication, adhering to organizational policies and standards.
