Service Area: Cook, Kane, DuPage, Lake, and McHenry Counties
Pay: $23-$25 per hour, plus mileage, tolls, and bonus opportunities
Schedule: Full-time, Monday-Friday daytime hours 7am-3pm

About the Role

You will provide compassionate, comfort-focused care to hospice patients in private homes, assisted living communities, skilled nursing facilities, and memory care settings.

Responsibilities include:

• Assisting with bathing, grooming, dressing, and personal hygiene
• Supporting mobility, repositioning, and safe transfers
• Providing companionship and comfort-focused care
• Observing and reporting changes in patient condition
• Completing accurate and timely visit documentation
• Communicating with patients, families, nurses, and facility staff
• Traveling between assigned patients throughout the service area

Service Area and Travel

This is a community-based field position supporting patients throughout a broad and varying Chicagoland territory, including Palatine, Schaumburg, Arlington Heights, Hoffman Estates, Skokie, Evanston, Rogers Park, and surrounding communities throughout Cook, Kane, DuPage, Lake, and McHenry Counties.

Reliable transportation, a valid driver's license, current auto insurance, and comfort with regional travel are required.
