MedZed Physician Services, LLC is seeking a Community Health Navigator II in Fresno. This field-based role combines in-person and remote outreach to engage Medicaid members, conduct assessments, and coordinate care planning with community resources. You will manage a sizable ECM caseload, document thoroughly in Salesforce, and maintain compliance with CalAIM and plan requirements.
