Description

• Assess patients in both inpatient and outpatient environments and develop personalized treatment plans
• Implement physical therapy interventions tailored towards enhancing mobility, strength, coordination, and functionality
• Collaborate closely with nurses, physicians, case managers, and interdisciplinary rehabilitation teams to maximize patient outcomes
• Guide patients and their families through therapeutic objectives, discharge processes, and at-home exercise regimens
• Maintain meticulous documentation of evaluations, treatments, and patient progress in line with facility standards and regulatory requirements
• Engage in case conferences and patient care team meetings to ensure cohesive and comprehensive care delivery

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory

Skills
