Client Stabilization Specialist

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency in Arizona providing residential, outpatient, inpatient, medical homes, medication‑assisted treatment, and crisis services for individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. Job Summary The Client Care Coordinator is a shelter or temporary lodging site‑based position focused on managing a housing caseload and coordinating care for individuals and families experiencing homelessness. The role includes coordinating and monitoring community‑based services, ensuring connection to providers, and facilitating delivery of services through the CBI Continuum of Care or other community agencies. Responsibilities also include completing data entry in the data system, assisting higher‑level personnel with reports, and preparing end‑of‑shift reports documenting care coordination. Skills & Requirements High school diploma or GED required. Associate degree (or higher) in a field related to Behavioral Health preferred. Minimum of six months of recovery from substance use and/or mental health disorders preferred. Minimum of six (6) months of paid related work experience providing exposure to behavioral and medical crisis situations required. 1–3 years of full‑time health care related work experience preferred. Certified Peer Support Worker or Recovery Coach certification required to be obtained within 90 days of hire. Valid identification required. Ability to pass a criminal background check required.
